A recent study  showed a significant weight loss in men who were obese. In this study men were on testosterone replacement for 8 years. Interestingly, the 8 year weight loss of 17-24% with testosterone therapy compares favorably to the 10 year weight loss of 14-25% seen after bariatric surgery!!
Considering that bariatric surgery is an invasive and expensive procedure that can only treat a minority of the rapidly growing obese population, non-surgical interventions that mimic the metabolic benefits of bariatric surgery, with a reduced morbidity and mortality burden, are tenable alternatives. To put this in context, the Look AHEAD Trial – which investigated the effects of an intensive lifestyle intervention with diet changes and exercise found a weight loss 6 % in the intervention group. Obesity drugs (orlistat, lorcaserin, naltrexone-bupropion, phentermine-topiramate, and liraglutide) only result in a short-term (1 year) weight loss of around 5%.
Hence, an accumulating body of evidence from long-term real-life studies of testosterone therapy in obese hypogonadal men suggests that testosterone therapy should be considered in the management of obese male patients.
Unfortunately we don’t have as many studies looking at women on testosterone replacement in regards to weight loss. However of late more studies are being done in women looking at testosterone replacement. I think we will see similar weight loss outcomes in women that we have seen in men.
